Family History
(Patients -> Open Chart -> Nursing Chart)
This section manages the medical history details of the patient's family. You may access, record, and change the details.
To access family history:
1. Use the main Search to search for a patient.

2. In the search results, click View Chart.
The Patient Chart is displayed with the Patient Dashboard selected.

3. From the left menu, click Clinical History.
The Clinical History window is displayed.

4. In the Clinical History window, in the Family widget, click Add/Edit.
The Family History popup is displayed.

5. Enter/Select the following details.
| S.No. | Field Name | Field Details |
| 1. | Relation | From the drop-down list, select the relationship of the family member to the patient. This is a mandatory field. |
| 2. | Health | From the drop-down list, select a health condition of the family member. This is a mandatory field. |
| 3. | Age | Enter the age of the family number. This is a mandatory field. |
| 4. | Alive | Select this check box if the family member is alive. |
| 5. | Reason of Death | If the family member is not alive, enter the reason for the death. |
| 6. | Diseases | From the drop-down list, select a disease that may be afflicting the family member. Then, click Add. |
6. Click ADD.
The family member’s details are displayed at the bottom of the window.
7. Click the ‘X’ symbol at the top right-hand corner of the Family History window to close it.
The Family section displays the entered details.
